WHAT DO YOU NEED TO REGISTER AS A NEW STUDENT AT LPHS?

1. Be living within the Lone Peak High School boundaries (or have your building permit, utility bill, closing papers, etc. to prove residency)

2. Have the required documents -
Proof of Residency
Copy of Birth Certificate
Transcript
Record of Immunization
Copy of Guardianship papers, if applicable
Blank forms filled out if available including your student's social security number

If you are a new student to the area or are considering moving within the boundaries of Lone Peak High School we welcome you to our truly world class high school! Please continue to read here. Lone Peak has established itself as a strong academic institution with an outstanding faculty. We are confident of our future. One of the things that you will notice about Lone Peak is the innovation and "cutting edge" technology associated with our curriculum. We have instituted an "Academy Model" that will facilitate a focus for better student learning and give renewed relevance to classroom instruction as it relates to the real world.

We are pleased that you are considering Lone Peak High as your school of choice. We hope this information will assist you in providing the information required to register at Lone Peak. If you consider our High School, the foremost important item is that you are living within our school boundaries. Lone Peak High is a closed boundary High School meaning that your students must live with a legal guardian within our boundary to attend Lone Peak . Our boundary includes portions of the towns of Highland, Cedar Hills and all of the City of Alpine. If you have any specific questions regarding your placement please contact the high school.

These are the items that we will need when you come in to register. It is essential that you have
all items to make your registration complete, without any delays.

A PROOF OF RESIDENCY. If you are building a home, you will need a copy of your building permit in your name. A copy of closing papers may also be used. If you are moving into an existing home, a copy of closing papers or a copy of a utility bill with your name on it will be required. If you are moving into a residence that is currently occupied (moving in with someone else) you will be asked to visit with Student Services at our District Office in American Fork. Their phone number is 801-756-8486. There will be a procedure that you will need to complete before any registering at Lone Peak, will occur.

A COPY OF YOUR STUDENT'S BIRTH CERTIFICATE. Before you move it may be wise to
make a photo copy of the original, since many times you may have this information stored away during your move. Please do not assume that a copy will be in your students records when sent to our school.

COMPLETE RECORD OF IMMUNIZATION. The State requirements for ALL students are:
(4) DPT, (3) Polio and (2) MMR's with the final shot of each immunization being given after the students 4th birthday. However, full immunization against Hepatitis A and B are also recommended.
If you a student that has been out of the country within the last 6 months you will also need to have a TB test and have it checked to be negative before you register. This test must occur after your student has entered into the U.S. Students must be current on all immunizations before they will be allowed to register or select any classes. A COMPLETE COPY OF YOUR TRANSCRIPT. This is essential, so that our counselors can build a class schedule based on the requirements that your student will need to graduate. The best source for this information, would be the school that you are leaving for a copy of your transcript and your immunization record.Generally, schools will not release an official stamped copy of a transcript, but should be able to give you a copy.

It is recommended that you contact us here at LONE PEAK when you know that you will be attending here, so that we can send you a packet of information, along with forms to be filled out. Please have your forms filled out before your appointment. This will save you time when you come in register.

Many of you may have questions regarding the District graduation requirements and courses that may be offered at Lone Peak. There are many options available along with many optional ways to earn credit. Most of your questions can be answered by accessing our web site (www.lonepeakhigh.com) then selecting "Courses Offered" along with various other buttons located at our Home page. If you will have a student that is not living with their legal custodial guardian , you will need to make an appointment with Student Services before registering. They may be contacted at:

If during your registration, a question may arise as to guardianship of your student, you may be asked to provide a copy of a divorce decree showing who has the legal and physical custody. Please understand that this is for the safety of our student body.
